Student Health Forms & Immunization Requirements

PLEASE READ these Guidelines for a Sick Child

Please read the guidelines for when your sick child should remain home and when they may return following an illness.

Health Forms

All completed health forms must be submitted to The Academy by September 1 unless otherwise noted. Please email completed forms to: forms@theacademysja.org

Illinois State Health Form

Due September 15
Required for:

  • All new students
  • All students entering Kindergarten
  • All students entering 6th Grade
Illinois State Vision Exam (Proof of Exam Required)

Required for:

  • Any student new to Illinois
  • All students entering Kindergarten
Illinois State Dental Exam (Proof of Exam Required)

Required for:

  • All students entering Kindergarten
  • All students entering 2nd Grade
  • All students entering 6th Grade
Student-Athletes

Any student participating in school sports must complete:

All students enrolling for the first time, including preschool students, must provide proof of required immunizations.

The Academy at St. Joan of Arc accepts only state-approved medical exemptions. All exemptions must be signed by an Illinois licensed physician.

Students who are not immunized or properly exempted will be excluded from attendance. The Academy reserves the right to require immunizations during unusual health circumstances, including public health emergencies.

Childhood Immunizations:

All children enrolling for the first time or entering pre-school are required to have proof of standard school immunizations. The Academy at St. Joan of Arc only accepts specified regulatory exemptions, which exemption(s) must be signed by an Illinois licensed physician. Students who are not immunized or properly exempted by a licensed physician will be excluded from The Academy. The Academy reserves the right to require immunizations for all students, regardless of exemption, during unusual health circumstances, such as pandemics. At this time COVID Vaccinations, while recommended for all who are eligible, are not mandated.
